Application Forms

The Application process to obtain a remote gaming license is divided into three stages.

First Stage: Fit and Proper

• Remote Gaming Licence Application Form (Form LGA/51/2010-01)
• Personal Declaration Application Form (Form LGA/51/2010-5)
• Personal Declaration Application Form - Key Official (Form LGA/51/2010-5-1)

Second Stage: Business and Technical Ability Assessment

• Business Entity Information Form (LGA/51/2004-3)
• A Guidance Checklist for Stage 1 and Stage 2 is advised to be done before attempting to proceed to the Third Stage.
• Second Stage Documentation Checklist is to be completed and submitted together with all Second Stage Documents.

Third Stage: Compliance Audit

• A Service Provider Authorisation Form (LGA/51/2004-4)
• LGA Compliance Audit Questionnaire - This is a requirement for those  operators seeking to gain or retain a license for Remote Gaming in Malta.
